Medical Alert Designation
Need Advance Notice of Planned Power Outages for Medical Reasons?
If you rely on electrically powered medical equipment, it’s important to register for
advance notification of planned outages. To do so, please complete a Request for
Medical Alert Designation. This form includes a Medical Certificate that must be filled
out by your licensed medical practitioner.
